Summary of the comprehensive assessment A nurse completes a comprehensive assessment on a patient and focused on the following social determinants of health and here are the findings: A 75yr old retired female patient presents with a history of hypertension, macular degeneration, sleep apnea, anemia. In addition, the patient has had knee surgeries and right ankle surgery. The patient has weakness in lower extremities and uses a walker to ambulate. The patient is retired and lives in a government subsidized housing and depends on social security to pay bills. The patients’ neighborhood has grocery outlets and no farmers market. The patient weighs 194lbs with a BMI of 32. After review of the health history, the nurse concluded that there are five priority areas of social determinants for the patient. 1) Quality of Educational status: patient completed 3yrs of college credits to get a job and did not graduate. The patient studied business and worked as a clerk. 2) Family support: the patient is single and lives by herself. Her son lives out of state and she rarely sees her family. The has a friend who is sick and on hospice. In essence, there is no social support system for the patient. 3)Transportation: the patient depends on friends to take her grocery shopping and appointments. 4)Socioeconomic condition: the patient has no income and social security pays helps the bills. 5)Neighborhood: the neighborhood offers grocery outlets, food desserts and no availability of farmers markets.
