Overview This module focuses on the competencies and functions of case managers. As the health care environment shifts and new influences surface to address the dynamic and complex needs of clients, case managers must possess competency in many skill areas to demonstrate effective case management practices at the individual, family, and population level. The process of case management is similar to the nursing process. Case managers are recognized experts whose practice is guided by a collaborative process which includes assessment, planning, facilitation, care coordination, evaluation, and advocacy (CSMA, 2010, p. 8). Case management practice is grounded in professional competencies, and this module allows the population health nurse to examine his/her individual circumstance to promote excellence in effective case management practice.
