This module will be assessed though a 3250 word written assignment which will be an assessment and plan of care for one patient discussed in class.

• Using the module assessment and care plan template ( Roper-Logan-Tierney Model of Living) write an evidence based assessment and plan of care using one of the scenarios from the module. This will include communication strategies and interprofessional collaboration.

• Explain the physiology of one of the blood pressure { vital signs }

• Discuss the importance of professionalism when caring for this patient.( NMC)

the scenarios

An old lady V. 82years old who has had a good life, she used to love planting flowers but her legs a bit wobbling now. She loves to cook for her husband S. steak pie was his favourite. Her pastry was to die for but she can’t do it any more because hands don’t move very well any more. She has lived in her hose for thirty-five years. With husband s. She really missed him. Her husband was in the Amy and died a couple of years ago. Now She lives alone in her house with a cat. One day While she was trying to pick the cat up from the floor she fell. She could not get up from the floor but managed to reach the phone and call her only daughter E. who lives down the road to come quick to help her out. Before the for fall she had a good health although she has hypertension which she is on medication for , apart from that her health was fine before the fall.

V. is an 82 years old lady who since the death of her husband lives alone in a small flat with her pet cat. V. has one daughter E. who lives nearby and visit s twice a week. V. has been blessed with good health before until now she does have high blood pressure which is been treated by medication prescribed by her general practitioner. It has been noted that V. has recently began to lose weight dropping from 60kg to 56kg in the last three months. One morning E. received call from V> her mum saying that she had a fall. E phone V> GP who visted later that day . V> had no injury as a result of the fall but her gP is concern about her safety and asked District nurses to visit and carry out an assessment of V> that After seeing her GP , the GP refer her to the community nurse who then need to carry out and assessment on her.

Module Aims

The aim of the module is to prepare the student to enter the practice environment through the exploration of core communication strategies, professional code of practice NMC code, interprofessional person centred care and human physiology as it relates to patient monitoring.

Learning Outcomes

By the end of this module students should be able to:

1. Demonstrate understanding of effective communication strategies

2. Discuss and demonstrate the importance of professionalism

3. Explain the normal physiology as it relates to vital physiological signs

4. Explore how to undertake a person centred assessment and plan care

5. Describe the multifaceted approach to caring for service user, including consideration inter professional working.

The students will study anatomy and physiology related to the measurement of physiological vital signs including pulse, respiratory function and rate, blood pressure and temperature control, students will also specifically examine musculoskeletal and gastrointestinal anatomy.
