The goal of population-based nursing is to provide evidenced based care to targeted groups of people with similar needs in order to improve outcomes (Curley & Vitale, 2016). This approach targets not just an individual, but all the people that populate a specific area having a tremendous impact on future generations. Individual focused interventions are targeted on the specific individuals challenge and independent situation. A holistic/biopsychosociospiritual (BPSS) intervention examines the five scales(impaired functional status, physical, psychological, social, and spiritual symptoms) necessary to view the full perspective of all the contributing variables that impact a persons life. Applying this perspective to an individual regarding their hypertension would allow a health professional to empower the individual to a new and different lifestyle.

Florida has conducted a comprehensive state health assessment utilizing Florida’s State Health Improvement Plan (SHIP) is a statewide plan for public health partners and stakeholders to improve the health of Floridians (Florida Health, 2018).
According to the Florida Health (2018), Cancer is one of the two leading cause of death, with more than 42,000 deaths each year and Florida ranks second in the nation in the number of newly diagnosed cancer cases. In 2014, nearly 7 out of 10 deaths were attributed to Chronic Diseases in Florida, this includes heart disease, asthma, diabetes, and arthritis. Behavioral health disorders are also on the rise, specifically substance abuse. In 2016, there was a rate of 14.4 deaths per 100,000 opioid-related overdose in Florida as compared to the national rate of 13.3 deaths per 100,000 persons. In the past several years, Florida has shown a dramatic increase in the number of deaths, particularly among those related to synthetic opioids (National Institute of Drug Abuse (NIDA), 2018). In Florida, there were 4,940 cases of Human Immunodeficiency Virus (HIV) diagnosed in 2016. As much as 127, 900 people may be living with HIV through 2015 (Florida Health, 2018).

Nursing informatics involves nursing science, information science, and computer science all designed to facilitate the management and process of nursing data, information, and knowledge with the primary aim to inform and support decision making as well as maintain delivery of care (Moen & Kundsen, 2013; Chamberlain University, 2018). Nursing information systems are supportive tools that enhance nursing operations including clinical, administrative and managerial activities (Farzandipour, Meidani, Riazi & Jabali, 2016). It is a tool designed to reduce errors, minimize costs and enhance delivery of high quality care for the patients and their families, in addition it enables the DNP-prepared nurse to use date efficiently and effectively in their daily nursing practice and in so doing develop and improve their knowledge to provide and improve patient care (Moghaddasi, Rabiei, Asadi & Ostvan, 2017
