Assessment consider what the aims of this stage are and how this was done for your patient through application of the model and risk assessment tools were relevant. Look at communication? Was the model applied appropriately? Was the focus on physical issues? How did they assess psychological issues? Did they use the model appropriately or was it due to their own experience and awareness of non-verbal cues?
Diagnosis was a holistic diagnosis given of actual or potential problems or was the focus just on physical care
Plan was this holistic and individualized? Did it follow from the assessment or just focus on the physical aspects of care to the neglect of psychological issues? Was the plan evidence based?
Implementation was this evidence based, individualized and holistic? Did it follow the plan, if not why not? For example was psychosocial care given but not documented.
Evaluation how was care assessed? What changes if any were made?
Conclusion: was the care episode effective?Were the model and process applied effectively? What were the strengths and limitations of the care delivery episode in relation to the provision of holistic, individualized evidence based care? How could it have been improved
