I browsed through the case study, my first decision would be to start Amitriptyline 25 mg po QHS and titrate upward weekly by 25 mg to a max dose of 200 mg per day, Explain why it is the most appropriate of the 3 proposed choices. My second decision will be to continue with Amitriptyline, and increase the dose of the medication to 125mg at bedtime, continuing towards a goal of 200mg, instruct also the patient to take the Amitriptyline an hour before bedtime, since the patient reported some improvement in symptoms, explain why not the other options, my third decision will be to continue with the current dose of Amitriptyline 125mg daily and refer the client to a life coach for exercise and dietary counseling ,Explain why you did not pick the other options.
